Free children’s immunization clinic set for Nov. 25
The free immunization clinic will be held from 1 to 4 p.m. Tuesday, Nov. 25, at the Assembly of God Church, 1020 East Maple in Centerville.
Immunizations offered are Dtap, Td and polio for children 2 months through 18 years of age. Prevnar is available to children 2 months through 11 months.
MMR is available to children 2 months to 5 years of age. The Hepatitis B vaccine is available to all children newborn through 18 years.
Varicella (chicken pox vaccine) is required for children entering kindergarten and enrolled in licensed child care.
Children entering kindergarten are also required by law to have the Hepatitis B vaccine series.
Please bring all immunization records with you. Immunizations will not be given without the signature of parent or guardian, and please have the child's or mother's Social Security number.
This immunization clinic does not take the place of regular health exams for your children by a doctor.
Immunizations are available for adults at the Appanoose County Health Department at the courthouse by appointment only. Call (641) 437-4332.
Donations will be accepted at the monthly clinic.
