CENTERVILLE —
The regular monthly immunization clinic is Tuesday, Dec. 18 from 1-4 p.m. by appointment only at Appanoose County Public Health office, 209 E. Jackson St., Centerville, Iowa 52544.
Childhood immunizations available include: DTaP, Polio, MMR, Varicella, Hep. A & B, Hib and Pneumococcal (Prevnar).
Adolescent immunizations available include: Td, Tdap, Hep. B, HPV (Gardasil) and Meningococcal (Menactra).
Flu vaccine is also available at our clinic.
Immunizations are free to children with Medicaid, children with no health insurance, and to children whose health insurance doesn’t pay for immunizations.
Bring all immunization records with the child. Immunizations will not be given without signature of parent or guardian, and mother’s social security number.
This clinic does not take the place of regular health exams by your doctor.
Immunizations are available for adults at the County Health Office by appointment only, call 437-4332.
Donations will be accepted at the monthly clinic.
